In the state of Florida, a written and notarized consent form is needed in order for minors to receive a tattoo or piercing. All minors must have a written and notarized consent form to receive a piercing (F.S.381.0075(7))*.
Minors aged 16-17 must receive permission from their parent or guardian via a signed and notarized consent form in order to receive a tattoo (F.S.381.0075(7))*
